Why WATTOLL

Connecting discharge with support at home.

A safe discharge depends on the right support being ready at home. Older people and families need clear plans, reliable visits and confidence about who to contact.

WATTOLL aims to offer NHS and local authority commissioners additional home-care capacity, connecting agreed discharge arrangements with practical support for recovery and independence.

The person’s goals guide every stage of support.

Our proposed services

Personal care.
Recovery-focused support.

Personalised support in people’s own homes, with clear responsibilities and care plans that reflect what matters to each person.

01

Personal care

Assessed assistance with washing, dressing, toileting, continence care and eating, protecting dignity and helping people do as much as possible for themselves.

02

Prescribed medicines support

Reminders, assistance and administration of prescribed tablets or liquids, delivered by trained, competent staff according to agreed care plans and recorded directions.

03

Reablement support

Help with daily living activities, mobility and agreed exercises. Community therapists retain responsibility for assessment, rehabilitation plans and clinical review.

Across the transition

Clear handovers. Reliable support. Planned reviews.

BEFORE THE MOVE

Agree the support

Confirm immediate care needs, goals, risks and responsibilities with the person and relevant teams before accepting a care package.

AT THE POINT OF TRANSFER

Put the plan into practice

Coordinate the handover and agreed home visits, provide planned support and escalate changing needs to the appropriate professional.

IN THE NEXT SETTING

Review progress together

Document progress towards agreed goals, inform professional reviews and plan reduced support or onward care as needs change.

Our intended impact

Better recovery for people.
Useful capacity for commissioners.

Safe and timely discharge

Aim to address delays linked to home-care availability once a person is ready to leave hospital.

Recovery and independence

Support daily living skills and confidence, working towards each person’s best attainable independence.

Continuity after discharge

Use clear handovers, reliable visits and timely escalation to help prevent avoidable breakdown in care.

Evidence for care reviews

Record progress and changing needs to inform reviews of ongoing support with the responsible teams.

These are intended benefits, not demonstrated results. Commissioned services would agree measures such as visit reliability, response times and progress towards personal goals.

Experience behind the service

Grounded in NHS experience.
Focused on independence.

The WATTOLL Team

WATTOLL is a team effort, drawing on leadership experience in NHS nursing, transitional care, discharge planning, Discharge to Assess (D2A) and community therapy liaison.

Our proposed service brings together care workers, operational management and coordination with community teams. At launch, nursing treatment and prescribing will remain with GPs, District Nursing Teams and other responsible providers.

We put the person, their relationships and what matters to them at the centre of every transition.

A few useful answers

Starting the conversation

Who will WATTOLL support?

Older adults needing personal care and recovery support at home, particularly following hospital discharge, illness or a fall. Care packages will depend on assessed needs and available staffing.

How could commissioning work?

An initial discussion can define geography, referral arrangements, staffing, response expectations and outcome measures. Delivery is subject to CQC registration, suitable capacity and an agreed contract.

Is WATTOLL providing care now?

WATTOLL is developing its service and welcomes enquiries. Regulated care will begin only after the necessary CQC registration. This is not an urgent-care service; availability must be confirmed directly.
